FINANCIAL POLICY

We offer several payment options and we will help you select which is right for you.  For your convenience, we accept cash, check, Visa, MasterCard, Discover and American Express.  We also offer a third party financing program through Care Credit, with interest free options for 6, 12 or 24 months. Financial concerns should not prevent you from obtaining proper oral health care.  Please contact our office at 847-882-9448 with any questions.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.  While we do file claims to your insurance company, you are responsible for all balances within 60 days of time of service.

At Schaumburg Oral and Maxillofacial Surgery we take pride in offering our patients the best in surgical care.  To make sure this takes place we do our best to work with you and your insurance companies, both medical and dental, to maximize your reimbursement for covered procedures.  We will file claims to your medical insurance company as an out of network provider.

We are participating providers with several dental insurance plans.  If you have questions please feel free to call us at 847-882-9448. 

Upon arrival of your first visit, please have your insurance information and picture ID so that we can expedite your reimbursement.

FIRST APPOINTMENT

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options.  In some cases surgery can be done the same day as the consultation. However, the doctor may decide to schedule a second appointment for your surgery if you have a complex medical history or an extensive surgical treatment plan.

Please assist us by completing your registration forms and by providing the following information at the time of your initial visit:

    -Your surgical referral slip and any x-rays

    -A list of medications you are currently taking

    -Your medical and dental insurance information

    -Your photo ID

Please note: All patients under the age of 18 must be accompanied by a parent or legal guardian.
